COVID-19 Discharge Planning and Care Coordination
Long-Term Supports and Services:
Long-term supports and services (LTSS) include care for people with disabilities who need support because of physical, cognitive, developmental, mental health, or chronic health conditions; or other functional limitations that limit their abilities to care for themselves.
Home and Community Based Services:
Home and community based services (HCBS) address the needs and preferences of people with functional limitations and are designed to enable people to stay in their homes and communities, rather than moving to a facility for care.
Supportive Services Programs:
Supportive services are community-based programs that address logistical concerns such as employment, transportation to medical appointments. They can include considerations such as accessibility in housing.
A Supreme Court decision that affirms individuals with disabilities are entitled to live in the most integrated setting and in the setting of their choice.
